Dear Parents/Guardians:
In the coming days we ask that you take a few minutes to go to the Parent Portal to complete these 4 MANDATORY forms for EACH of your children at school. These forms must be completed again each and every NEW school year.
Sign into the Parent Portal and select ‘FORMS’ from the menu on the left. Here you will find the following forms to be filled out:
Consent for Use and Disclosure of information - This form is important for all students, as staff will take many photos and videos for school plays, assemblies, special events and slideshows to name a few. Permission/consent to take photos/videos of all JK to Gr 6 students is important. Classroom teachers DO VERIFY the list of who has consent and who does not to help with their planning.
Responsible Use of Technology - This is for your child to have permission to use a chromebook/technology at school. They may be restricted to use a school device until this form has been properly completed.
Off Site Permission Form - This form gives the teachers permission to use the field/park that is off school property, walking paths close to the school, local library and other nearby areas that are all within walking distance of our school.
OCSB Student Verification - It's important for the contact information on file at school to be up-to-date. If any of your contact information (phone number, email address, home address, etc.) changes, the easiest way to update it is to use the OCSB Student Verification Form in the Parent Portal. For our little ones, it is especially important that you indicate who has permission for school pick up by checking off the appropriate box beside that individual's name. After a public consultation process, Trustees approved an option attendance area for Chapel Hill and St. Kateri Tekakwitha Schools at a Board meeting on January 28, 2025.
These approved changes to attendance boundaries for the schools will be effective for September 2025.
A brief summary of the recommendations which were approved, are as follows:
Creation of an Option Area that would permit elementary students residing within that portion of the St. Kateri Tekakwitha attendance boundary located south of Innes Road to attend either St. Kateri Tekakwitha or Chapel Hill School.
This Option Area would be effective beginning in September 2025.
Should a part of the Option Area be redirected to the New Mer Bleue Catholic Elementary School, determined through a future consultation, the remaining portion would continue as an Option Area.
Existing students at St. Kateri Tekakwitha living within the Option may remain at the school, however the option to attend Chapel Hill is available for those families that choose to do so.
St. Matthew High School will remain the designated secondary school for students attending both Chapel Hill ES and St. Kateri Tekakwitha ES.
